New Patients

 All new patients must complete the following:

  • Patient Information Form
  • Bring Insurance Card 
  • Any Previous Medical Records

Appointment Arrival Time

 Please arrive 30 mins early to complete paperwork and/or tablet questionnaire. Patients have the option to complete the questionnaire ahead of time through the patient portal. 

Cancellation Policy

 We require a 24 hour advance notice of cancellation.  You will be responsible for paying for the visit if appointments are canceled with less than 24 hours notice prior to your scheduled appointment, or if appointments are not kept.    Exceptions are made for late cancellations/missed appointments due to emergencies.  You will need to discuss exceptions with your provider.  Please note that insurance companies do not cover the costs of missed appointments.

 Once you have scheduled your appointment you will receive an email with your log in information to access the patient portal. From there you will be able to:

  • Ask questions to staff
  • Request prescription refills and referrals
  • Examine your current and past statements

If your doctor has already sent in a referral to our practice please contact us to schedule an appointment

If you are a referring provider sending in a new referral, please fax it to 919-278-2042 and our staff will contact the patient.

INSURANCE TIPS

  •  Ensure that your contact information and address is current with your insurer and our office
  • Please notify us about any changes with your insurance at each visit
  • Always bring your insurance card to your appointment
  • Know if you need a referral prior to scheduling
  • Know your Mental Health benefit plan and co-pay/deductible amount 
  • If we do not participate with your insurance, contact your insurance carrier directly to see how to file your own claim
